Service Description: Adult day program * social, recreational, and therapeutic activities in a group setting for part of the day * helps to support family caregivers who provide day-to-day care

  • computer activities
  • games and trivia
  • physical games such as bowling and bocce
  • care planning



Service Details:

Transportation: York Region Transit. Hand to Hand Transport, contracted service provider at minimal cost to client

Nutrition: light breakfast, healthy lunch and nutritious snacks 

Mobility Assistance: minimal assistance, must be able to ambulate independently

Incontinence: discharge from this program

Medication Management: medication cueing/reminders * medication must be brought in a pharmacy created blister pack and include only required medication for program hours with appropriate information labelled

Fees: $26.49 per day * subsidy available for eligible participants
