The Central Community Care Access Centre
(CCAC) is one of 14 CCACs that play an important role in Ontario’s health
system by coordinating quality in-home and community-based care for over
650,000 people each year. 1.8 million people live in the highly diverse
communities of South Simcoe, York Region, North York and areas of northern
Toronto that are served by the Central CCAC.
Central CCAC has three main offices
located in North York, Richmond Hill and Newmarket. Central CCAC staff are also
located onsite at North York General, Humber River Hospital, St. John's Rehab,
Markham Stouffville Hospital, Mackenzie Health, Southlake Regional Health
Centre and Stevenson Memorial Hospital.
